Description

The Forest at Duke recently opened our newly-constructed, state of the art, spacious 90-bed Health Center. Utilizing the Small House concept, the new Health Center features nine "small houses" that support varying levels of care within our established continuum: Assisted Living, Memory Care, and Skilled Nursing.

 

The Activities Care Partner assists in implementing and evaluating a program of life-enriching activities that is based on an ongoing assessment of residents’ individual needs, ability levels, and interests. They work in work in collaboration with the house care partner team to manage the residents’ environment, including cooking, dining service, laundry and housekeeping, and basic (non-certified) resident care.

Schedule: Full-Time, 40 hours a week, some weekends required

Key Responsibilities:

  • Assists in designing and implementing a balanced schedule of individual and group activities for health service residents based upon the therapeutic recreation plans that have been developed for each resident.
  • Assists in planning a diversified goal-oriented therapeutic activities program which will contribute to the achievement of meaningful, measurable goals that have been adapted to the ability levels of each participant and is structured to maximize the therapeutic benefits to the participants.
  • Uses community resources, encourages patient/resident community involvement and participation on trips.
  • Participates in cooking, housekeeping, activities, and basic (non-certified) resident care in partnership with the house care team.

 

Key Requirements:

  • High School diploma or equivalent preferred
  • At least one year of related experience in healthcare setting preferred
  • Must be able to understand and communicate orally in English.
  • Must be at least 18 years of age.
  • Must complete North Carolina Food Handler Class within the first 90 days of employment.
